Each of our 24 clinics had grown up independently, and each one had its own system for tracking referrals and patient intake. Some used spreadsheets. Some used a CRM nobody else in the network had access to. When a patient was referred from one clinic to another, which happened constantly, the referral information often had to be re-entered by hand or faxed across.
We knew we needed one system. What we did not know was how to get there without losing years of referral history and patient intake records that our clinical operations teams depended on every day.

Patient referral and intake data was fragmented across 24 clinic locations, spread between a legacy CRM, standalone spreadsheets, and in some clinics, paper intake forms scanned into shared drives. There was no unified view of the referral pipeline across the network, which meant leadership could not see where referrals were getting delayed or lost between clinics. Any migration had to preserve sensitive, HIPAA-relevant fields with complete accuracy, and the network could not tolerate any gap in patient-facing operations during the transition.

How we thought about it
Given the sensitivity of the data involved, we treated data mapping as its own project phase before any migration began. Every field across all 24 legacy systems was catalogued, classified, and mapped to its destination in HubSpot before a single record moved. We ran the migration in stages by clinic rather than all at once, with a parallel-run period for each clinic where both the old and new systems operated side by side until the data was independently verified as complete and accurate.

We ran a full HubSpot onboarding for the network, with a field-level data mapping and validation process covering all 24 clinic systems. Migration was staged by clinic, with automated reconciliation checks comparing record counts and key field values between the legacy systems and HubSpot before each clinic's old system was retired. On top of the migrated data, we configured Marketing Hub, Sales Hub, and Service Hub to give the network a single, unified view of the referral pipeline for the first time, along with structured intake workflows that replaced the manual re-entry process between clinics.

What actually changed
The migration was completed with zero patient records lost across all 24 clinics, verified through the reconciliation process at every stage. Referral response time between clinics improved 3.5x now that a referral raised in one clinic is immediately visible to the receiving clinic without manual re-entry. The network's leadership team gained visibility into referral pipeline health across all locations for the first time, and the migration was completed without any interruption to patient-facing operations at any clinic.
